Loader Crane Over in Manchester

Manchester, England

An articulated loader crane overturned while carrying out a lift at a pub in Manchester on Thursday night.

The crane, mounted on a articulated truck/tractor trailer, was delivering a large advertising hording, it had lifted it clear of the trailer slewed over the side and was moving it into place when it went over. There were a few patrons in the pub- the Angel – at the time, but while part of the building was damaged no one inside was affected.

It is hard to see if the overturn was caused by an issue with the outrigger set up, soft ground conditions or a simple overload. Thankfully no one was injured in the incident.

Telehandler Over

We have received a report that a telehandler overturned yesterday in Melbourne, Australia, but have no details of where it occurred and what it was doing.

It looks as though it might have been travelling up a slope with a fully extended boom when it overturned. We do know that no one was reported to have been hurt in the incident.

Crane Roll

Scotland,

A six axle All Terrain crane left a single track road and rolled into a field in Scotland earlier this week

So far we have been unable to trace the exact location of the incident, which involved a six axle Liebherr All Terrain owned and operated by Ainscough Crane Hire. We understand though that it occurred on one of the single track roads around Loch ness to the south of Inverness.

The driver was not seriously hurt, but did suffer cuts and bruises. A recovery team has now arrived, – with wrecker trucks, a larger loader crane and earthmoving equipment, not to mention airbags- and has been trying to pull the crane back up onto its wheels.

Fatal Crane Overturn in Alabama

Parrish, Alabama

A crawler crane overturned on a highway project near the town of Parrish, northwest of Birmingham, Alabama yesterday killing the operator.

The crane was working from a terrace on a sloping job site and looks as though it was preparing to tandem lift some concrete bridge beams, but the boom went over the back of the crane causing it to overturn. The operator sensing the crane start to go jumped out of the cab, but the crane then landed on him. He was declared dead at the scene.

That is all the information that we have at the moment.

Crane Overturn Delays Refinery Sale

Torrence, California

A 270 ton lattice truck crane overturned yesterday at the Exxon Mobil refinery in Torrance, near Los Angeles, California.

The crane was reported to be lifting a reactor top for a 100,000 barrel a day fluidic catalytic cracking unit, although a photo we have been sent suggests that it was preparing for the lift, rather than carrying it out at the time. The six axle crane looks to be short rigged and tipped over backwards, causing the boom to fall onto part of the refinery. Three men working with the crane received minor injuries.

The damage and time needed to clear it is expected to delay the refinery from reaching 15 straight days of normal operations, as required by a sales agreement with PBF Energy, which is paying $537.5 million for the plant.

PBF spokesman Michael Karlovich said: ”PBF is working with Exxon to understand the impact of today’s incident on refinery operations. We continue assessing Torrance refinery operations and will update our prior statements on the transaction if and when appropriate.”

Crawler Overturns on Soft Ground

Plaquemine, Louisiana

A 600 ton crawler crane overturned last month at the Dow Chemical plant in Plaquemine, Louisiana.

Information on this incident have only recently been received, but have now been confirmed, it seem the incident occurred on Thursday September 8th while a relatively new Liebherr LR1600 was being moved while fully rigged.

The crane, reportedly owned and operated by Mammoet, was travelling on timber mats, arranged in a loose longitudinal pattern. The pressure at one point proved too great and the timers and crane track sunk into the soft ground causing the overturn. We have been assured that no one was injured in the incident. Plaquemine is to the south of Baton Rouge in Bayou country.

Dow said that it is working alongside Mammoet to investigate the incident to find out what caused the crane to overturn.

Crane Overturns in Texas

A Rough Terrain crane overturned in Arlington, Texas yesterday pulling overhead power lines down onto the Highway.

A Rough Terrain crane overturned in Arlington Texas yesterday pulling overhead power lines down onto the Interstate 30 highway halfway between Forth Worth and Dallas. The crane, a larger Lorain, was working on a road job constructing a new overpass when it overturned. Fortunately no one was hurt, although the highway was closed in both directions while the power lines were properly deactivated.

The crane’s outriggers were correctly set up with mats under the pads. It looks as though the cause must be down to overloading, although we do not know what it was lifting at the time, but photos show that the boom came down into an open top shipping container. We will update when we learn more.

Double Crane Fatality

Tamil Nadu, India

Two bystanders were killed on Friday when an articulated pick & carry crane overturned while trying to pull a crashed truck off the median of a divided highway in Coimbatore in the state of Tamil Nadu in southern India.

The crane started to lift the truck when it rolled forward applying a side loading to the crane, pulling it over. The two men , a painter (32), and a night watchman (66), are reported to have died the instant they were hit by the falling crane. Two similar cranes were called in to carryout a hairy looking recovery of the overturned crane.

Two Injured in Crane Overturn

A crane operator and a homeowner were injured yesterday, in Lauderhill, near Fort Lauderdale, Florida, after a large crane overturned onto a number of houses.

The crane, a five axle All Terrain owned and operated by local rental company Sims Crane & Equipment was working with full boom extended, placing power poles to the rear of the row of houses for Florida Power and Light, when the nearside outriggers appear to have punched through the road surface into a water main.

The boom badly damaged a number of houses and brought down power lines, but fortunately the injuries that the two sustained are not thought to be too serious. As far as we can tell the crane was set up on medium sized circular outrigger mats, although a truck parked alongside the crane appeared to have four large rectangular steel mats on board.

Crawler Crane Overturn Captured on Video

A crawler crane rigged with long boom and jib overturned on the construction site of the new police headquarters in St Petersburg, Florida on April 5th, 2018, narrowly missing a number of men working below.

The crane, a Terex branded IHI, was lifting an electric scissor lift onto the upper floor of the building when it appears to have gone beyond its safe radius and lost stability. The scissor lift landed on the front of a parked telehandler, while the boom struck its rear end. The main contractor for the project is reported to be Ajax Building Corporation.

The overturn was captured on Police Department surveillance cameras and shows those on the ground running for their lives as the boom came down. Thankfully no one was injured in the incident, although the crane operator was reported to have limped away from the crane after climbing out of his cab.

Tree Takes Out Crane

A truck crane overturned in North Smithfield, near Providence, Rhode Island on Monday, June 27, while lifting trees from the back yard of a home.

One of the crane’s rear outriggers appears to have given way, causing the overturn. As the crane tipped over, one of the front outriggers smashed though the side of the home, entering a room that was occupied at the time. Thankfully no one was hurt in the incident.

The crane, a new Altec, was owned and operated by Mikes Professional Tree Company, and had only been delivered a month ago. The company blamed the incident on an equipment fault stating that a “piston on one of the rear outriggers had failed causing it to collapse in on itself, resulting tin the crane overturning” although the crane was quickly recovered – with the help of another crane and winch to pull the front end back down – the boom and outriggers were fully stowed without incident before it drove away under its own power to be repaired.

OSHA is investigating the incident.

Tower Crane Overturn

A tower crane overturned onto the roof of a house that it was helping to renovate in Neckargemünd – west of Heidelberg, Germany on Wednesday afternoon, July 27, 2018.

The self-erecting crane, an Arcomet T25A, was set up on the hill side and was being operated by remote control when its outriggers appear to have slipped off of a beam they were set up on, dropping into the foundations of the building causing the crane to overturn.

Fortunately, the operator was well out of the way at the time and the house was unoccupied, so no one was hurt. Once a technical accident inspector had checked out the scene a mobile crane was called in to remove the wreckage from the house.

Fatal Crawler Crane Overturn

A crawler crane operator has died in an incident at a wind farm in Poweshiek County, Iowa over the weekend.

Blattner Energy, which is currently installing turbines for MidAmerican Energy at the North English wind farm in Poweshiek County– has confirmed that a crane operator died as a result of an incident on the project but has not released any details.

We understand however, that the crane was raising a long boom when the ground gave way under the tracks. The boom landed back on the ground with the impact causing the counterweights to crash into the operator’s cab. Although this has not been fully corroborated. We will update this as soon as we have more information that we can use.

The Poweshiek County Sheriff’s Office and OSHA are investigating.

Tree Takes Boom Truck in Chatanooga

A boom truck owned by a tree care company overturned onto a house in Chattanooga, Tennessee while working to remove a tree from the front yard.

The crane was owned and operated by Big Woody’s Tree Service, thankfully no one was injured in the incident, although some significant damage was done to the roof and front façade of the house. A photo we have received suggests that the rear outriggers were not fully extended, but they may well have retracted as the crane tipped.

The boom truck was later recovered by Doug Yates Towing, using a heavy duty long boomed wrecker.

Fatal Crane Incident

A brand new 1,250 ton Kobelco 16000 J-H crawler crane collapsed while undergoing final tests on July 26th at Kobelco’s plant in Takasago, Hyogo Prefecture, Japan.

The boom, jib and derrick mast collapsed sideways striking a number of buildings and vehicles in the vicinity. A man working in the immediate area, Tadashi Yamaguchi, 56, was declared dead at the scene, while Shihei Nishida, 23, secumbed to his injuries and died in hospital. A third man aged 59, remains seriously injured as does a fourth man. Others from the photos and many videos posted locally, it looks like the derrick mast was ripped off its pivot pins, while the boom failed at the connection with the butt section.

UPDATE
Kobelco Construction Machinery has now issued a more detailed explanation of the incident which is published in full below:

“On July 26, an SL16000J-H crawler crane manufactured by Kobelco Construction Machinery Co., Ltd. was undergoing a load test prior to shipment.
The load test, where a suspended load is hoisted, was being conducted at Kobelco’s test yard, which is located within Kobe Steel’s Takasago Works* in Hyogo Prefecture in western Japan.
(*Note: Takasago Works is a manufacturing complex for Kobe Steel’s business divisions, covering steel and other metals as well as industrial machinery and equipment.)
When the crane was swinging during the test, for some reason, the boom and jib collapsed sideways, breaking the entire attachment.

The accident caused the death of two people and injured two others, a total of four. All four were doing other tasks when the attachment fell.
With a lifting capacity of 1,250 tonnes, the SL16000J-H is one of Japan’s largest lattice boom crawler cranes. The crane went on sale in Japan in May 2016. The cranes that have been delivered to customers have all passed the tests and are currently in operation.

Why the accident happened is unclear and the cause is currently under investigation. Kobelco is cooperating fully with the police and other authorities in the investigation in an effort to determine the cause.”

Kobelco company officials also also issued the following statement: “We sincerely apologise for the loss of two precious lives (the two people from our subcontracting company), and deeply regret causing this serious accident,”

“We offer our deepest sympathy and condolences to the bereaved families of the two who passed away. In addition, we pray that the two injured workers will make a speedy recovery,” orking in the area or the buildings affected received minor injuries.

Crane Overturn

A large All Terrain crane overturned in the UK September, 11, at a new energy from waste installation in Kent.

We understand that the crane -a Liebherr from the Ainscough fleet had – developed an issue with the cable reel for the extension over the weekend and was left in place overnight with the boom raised and bi-fold extension in place. As the crane was set up alongside a walkway, the outriggers on one side were short rigged in the fully retracted position. Due to the size of the counterweight the correct boom and slew position were selected for maximum stability overnight.

We understand that the operator and mechanics arrived on site yesterday morning to sort out the problem. However… it seems that the operator raised the boom and then slewed the crane, taking the large counterweight out over the retracted outriggers causing it to tip over backwards – indicating that the overload system was probably not set up for the short rigged configuration.

The boom and extension came to rest on some newly installed tanks, thankfully no one was hurt in the incident and it looks as though damage might also be relatively modest. It could have been much nastier if anyone had been walking past the crane at the time.

This looks like a typical operator error, one that we have reported on numerous times over the years, and usually occurs during setting up or stowing the crane, when the operator forgets the short rigged outriggers behind him, raises the boom and then slews the counterweight out over the retracted outriggers.

Crane Mishap in Scotland

A telescopic crawler crane tipped over rearwards October 19, 2018 at a site in Edinburgh, Scotland.

The crane, a 100 tonne LTR1100 owned by Hawks Hire, was working on the St James Shopping center and hotel redevelopment when it seems the operator had the tracks fully retracted, over the side with full 32 tons of superstructure counterweight in place, and then either raised the fully retracted boom to the point where the crane became back heavy and tipped or slewed into that position?

The crane does not have a 360 degree chart with the 3.5 meter track width. Fortunately, the crane was close to the building and the boom came to rest against the structure. No one was hurt – apart from the operators pride perhaps – and the damage looks to be minimal.

 

 

We have no information on whether the crane operator was employed by the contractor or the crane owner. And assume that it was in set up mode or a wider track width was programmed into the crane management system.

*UPDATE*

The following statement has been received from Hawks managing director Elliot Hawkins following a personal visit to the site:

“Hawks is currently working with the manufacturers and investigators to determine why the crane tipped over. We are grateful that no-one was hurt in the incident. At present it is thought an error occurred during derigging and rigging the crane resulting in it toppling over.”

“Friday and the days following the incident in Edinburgh have been a difficult time for both us as a company and for the individual operator. This is the first incident of this type we as a company have been involved in during our five years of operation, a testament to how seriously we take health and safety regulations on all the work we undertake.”

“The accident has been a big shock to the team working in Edinburgh and those at our headquarters in Dartford. At present we do not know the definitive reason why, or how the crane tipped over. We continue to support the operator, who has provided many years of loyal service to Hawks and will continue to help him in whatever way he needs to overcome the shock of being involved in this incident.”

“Once the investigations are concluded we will be able to comment further, but until such time we will not be making any further statement or answering any further questions that could prejudice the outcome of these investigations.”

Boom Truck Overturn

A man was injured last Thursday, November 6, when a boom truck/platform he was using overturned in Homestead, on the south side of Miami, Florida.

The man was operating the machine as a crane from the truck bed, using the lower controls when it tipped over, he managed to jump clear but injured his back in the process. He was treated by emergency services at the scene before being airlifted to a local hospital.

The cause has not been revealed, but seems to be outrigger related although we cannot confirm that. The boom of the machine, which is owned by Acolite Claude Sign, was cut into sections during the recovery process, a small fuel and oil spill was contained by the emergency services.

Another Tele Crawler Overturn

A 70 ton Sennebogen 673E telescopic crawler crane tipped onto its counterweight yesterday, the third case of a telescopic crawler overturning in this way in the UK in the past two months.

This particular incident is said to have occurred on a job site alongside Luton Airport where work has just started on updating the rail link. The crane has its tracks fully retracted with the retracted boom fully elevated and over the side with full counterweight in place. In which position it is almost guaranteed to tip rearwards. The soft ground on site may also have contributed to the incident.
